Research referrals will be sent via the information system to offices of all voivodships, regardless of the citizen’s place of residence. Based on the results of the study, after making a decision, the citizen’s personal data will be reflected in the final document — a disability certificate indicating the group and individual rehabilitation program of a disabled person.
